Medical Foster Home
Tomah VA Medical Center is pleased to announce the development of the Medical Foster Home Program (MFH). A MFH is an adult foster home where Veterans who are unable to live independently can live in a family like setting under the care and supervision of a caregiver. MFH placement is voluntary. The MFH is chosen by the Veteran and matched with the Veteran’s specific physical, social, and emotional needs. The Medical Foster Home program staff can help you find a MFH that provides 24 hour supervision, protection, and quality care. MFH is a wonderful alternative when living alone becomes difficult.
The MFH program provides comprehensive case management visits by VA Home Based Primary Care Home Care Team: (Physician's Assistant, Nurse Practitioner, Social Worker, Dietician, Psychologist, Occupational Therapist & Recreational Therapist).
